I agree and understand that by checking this box that I do hereby give my child permission to attend Mt. Zion Missionary Baptist Church VBS 2024. In consideration of the privilege of allowing my child to participate in above-named activity, I agree to release and hold harmless Mt. Zion Missionary Baptist Church, or any of its members, from any liability to or responsibility for bodily injury, damage or illness to the above-identified child while participating in child athletic or social activity which may directly or indirectly sponsored by the Church. Further, I agree to indemnify and hold harmless the Church, or any of its members with respect to any claim asserted by or on behalf of my child as a result of bodily injury, illness, or damage.
By selecting yes, I grant permission to Mt. Zion Missionary Baptist Church, its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child, myself, and members of my family while at Mt. Zion Missionary Baptist Church for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release Mt. Zion Missionary Baptist Church and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use the statements of my child, myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, my child’s rights, and my family’s rights to any and all compensation stemming from the use of these materials.”
